When was the tank set up?

When/where did you get this fish? Any previous health problems?

What type of water do you use? Do you treat the water with dechlorinator or add anything else to the water routinely?

What other fish live in the tank (species, number). Any sick or recently died? Any new added recently? Any aggression between them?

What kind of filtration do you use? What do you do to maintain it (such as changing/cleaning the filter media) and how often?

What temperature is the tank water? How is it maintained (heaters, chillers, etc)?

How often do you change water? How much do you change each time?

What do you feed, approximately how much and how often?

Have you done anything yet to try and treat this problem (added meds to water, changed temp etc)?
